Request for Employee Records

This online form is for use by applicants and current/former employees of Lamar CISD to request records from their personnel file (i.e., transcripts, teaching certificate). After completing the form and clicking the "submit" button at the bottom of the page, your request will be emailed to the LCISD Human Resources department. Requests will be processed in the order that they are received. Please allow 10 working days for all record request(s) to be processed.
